What is US Customs Form 6059B?

How to complete a U.S. Customs Form

Complete the Front of the U.S. Customs Form
First, complete the front. Be aware that only one family member can fill out the form. This means that any traveler arriving should fill out the form.
You will need to provide some basic information about yourself. With that in mind, you must fill out the first section with your last name (family) and first name (given). The first letter of your middle name should be printed.
Your birth date should be entered in the appropriate boxes for month/year. You should also mention the number of family members who are traveling with you. Do not include yourself in this number.
Next, enter your street address in the United States. It’s also important to note that if you don’t have an address or are staying in a hotel, you should mention it. Enter the address of where you are staying. Print the name of the state and the city in the boxes. Add the name of your country and your passport number.
